The purpose of this program is to support health departments in implementing pre-exposure prophylaxis (PrEP) and Data to Care demonstration projects. Projects must adopt one or two approaches. Category 1 is for PrEP projects prioritizing men who have sex with men (MSM) and transgender persons at high risk for HIV infections, particularly persons of color. Eligible activities for this category include training clinicians about PrEP and develop policies that support PrEP as an HIV prevention tool. Category 2 is for Data to Care projects that use HIV surveillance data that prioritize identifying HIV diagnosed MSM and transgender persons who have sex with men. Eligible activities include developing organization policies and protocols to use HIV surveillance data and building capacity to implement Data to Care activities.

Eligibility Notes
Eligibility is limited as follows:
Category 1: Health departments whose jurisdiction includes one or more of 32 Metropolitan Statistical Divisions in which there are greater than 3,000 MSM living with diagnosed HIV.
Category 2: Health departments that are eligible to apply for Category 1 and have complete reporting of all
CD4 cell count and HIV viral load results.
